What is it?
Continuous Care Programs help support patients between office visits. Patients are given a dedicated registered nurse to help navigate their health. Patients can connect with their team from the comfort of their own home. No more waiting, worrying or trying to figure it out on their own.

Dedicated Registered Nurse
A dedicated nurse will advocate for patients' needs, coordinate with other providers and support their health goals
Convenient Access to Care
Patients can call their nurse directly with questions or concerns about their health.
Personalized Care
The Registered Nurse will work to address patients' health needs quickly and guide them to better health
Services include direct access to a Registered Nurse, easy to use devices, personalized care plans, early intervention and care, and education and support.
Programs
315 health offers three programs for patients to enroll in. These services are available for patients living with chronic conditions such as high blood pressure, diabetes, congestive heart failure, obesity, ulcerative colitis, Crohn's disease, and so forth.
Remote Patient Monitoring (RPM)
In-home devices used to track patient's health provided by 315 health. Patients' dedicated Registered Nurse will work with them on tracking their readings, and providing care using those readings.
Devices are matched with patients based on their individual conditions.
Chronic Care Management (CCM)
Care management support for patients with at least 2 chronic conditions.
Principal Care Management (PCM)
Care management support for patients with at least 1 chronic condition. (Ideal for specialists)
Patients enrolled in either CCM or PCM will be enrolled in RPM
